Sweet Grass Community Health is here to bridge the gap between you and the services you need.  Whether you are in need of a therapist, a ride to get to your next doctor's appointment, or help filling out your Medicaid application, one of our Community Health Workers would be happy to assist you, and if for some reason, we are not able to help, we will get you pointed in the right direction of someone who can.
